Evaluation and Desk Copy Requests

Note: You must be an instructor at an accredited vocational career center, community college, or university to be eligible to receive an evaluation copy or desk copy.

• If you would like to consider a book for classroom use, please complete the online evaluation copy request form below.
• If you are requesting a desk copy, please do not complete this form but refer to our desk copy policy below.

Desk Copy Requests
Please mail or fax requests for desk copies, on school letterhead, to:

In your request, please include:

1. The course name and number
2. When the course will be taught
3. The name of the professor(s)/instructor(s) teaching the course
4. Confirmation that your college bookstore has ordered a minimum of 10 units for use as a required text
   (PLEASE NOTE: if you don't provide proof of purchase, we'll send you a desk copy with a billed ninety-day (90) invoice. We will cancel the invoice on notice that an order for at least ten copies is forthcoming or if you return the book. We will provide one (1) desk copy per ten (10) units ordered by the bookstore.
5. The name of your college bookstore or purchasing office (if available)
6. The date of the order (if available)
